Number 07-09: Tako-tsubo or Infarction
Case from: Ajay Yerramasu
and Anthon Fuisz. Washington Hospital Center, Washington DC, USA.
History (A): 65 female presented with an apparent acute MI
following emotional stress. Unremarkable coronary angiography (A2).
History (B): 40 female presented with an apparent acute MI.
Angiography demonstrated acute LAD lesion (B2).
CMR: Apical hyopkinesis is seen in both cases (A1, B1). CE-IR delayed enhancement
present in case 2 only (B3).
Comment A: Absence of scar after stress suggests tako-tsubo cardiomyopathy.
Comment B: CE-IR images confirm myocardial infarction.
Perspective: The absence or minimal presence of myocardial necrosis in tako-tsubo
can be confirmed on CE-IR MRI with delayed imaging.
